• Complete the Be A Mentor Online Application
• Complete Mandated Reporter Training annually
• Tuberculosis (TB) Clearance Risk Assessment
• Driver Transportation Form (Level 3)
• Auto Insurance Coverage Declaration page (Level 3)
• All volunteers must receive notification of clearance from Be A Mentor before they can begin volunteering

Volunteer  Level 2	Provides regular volunteer service without transport duties	Does not supervise students without a district staff member present.	Regular classroom or library helper, lunch helper, field trip chaperone (under supervision and without driving). Volunteer  Level 3	A volunteer authorized to supervise and transport students.	May supervise students while driving them to an activity without a district staff member present.
